• Citizenship certificate (both sides)
• Parents current passport

To assist with completing the overseas application form, we have provided some pointers below:
1. Page 1 - Personal details
This page should be completed using the information on the applicant’s birth certificate and citizenship certificate. When you enter the citizenship certificate number at section 3, please use the “ACC” number printed on the reverse of the Citizenship Certificate or the “CBD” number on the Citizenship by Descent certificate.
2. Page 4/ 84F - Consent
When completing this page, please input the information as per the applicant’s birth certificate.
For example, the details should be completed as mentioned below:
13a - Parent 1’s name as mentioned on the child’s birth certificate
13b - Parent 1’s name as mentioned on their passport
13c - Parent 1’s Australian Passport details (if applicable)
14a - Parent 2’s name as mentioned on the child’s birth certificate
14b - Parent 2’s name as mentioned on their passport
14c - Parent 2’s Australian Passport details (if applicable)
You will also need to ensure Parent 1 and Parent 2 sign this page and have it correctly witnessed/ dated. Please ensure witness should not be related to the applicant/ family.
3. Parents proof of identity
Copies of both parents’ identity documents that show their name, photo, signature, and current address (i.e., both parents and those with parental responsibility for the child who signed section 17 on the application form)
Name changes: If the current names of either parent are different to how they appear on the child’s birth certificate, please include certified documents that explain the change of name.
4. Signatures and dates
Before submitting the application, please double check you have signed and dated the form (wherever applicable). You should also check the guarantor and witness have completed their sections in full. Dates should be entered in Australian format (dd/mm/yyyy) and should not be more than six months old.
5. Photos
You will need to obtain new photos if the photographs are over six months old. Further information about photos is available here: Photo guidelines | Australian Passport Office (passports.gov.au)
6.Payment
